Notify or update us about reportable allegations under the Reportable Conduct Scheme

Before you begin

For best performance we recommend using the latest version of Google Chrome. 

Organisations must notify the Commission within three days of the head of the organisation becoming aware of a reportable allegation by using this webform.

Members of the public can also notify us about allegations of reportable conduct by making a public notification using this webform or by phone, email or letter.

The Commission has a range of guidance about the Reportable Conduct Scheme.

There are also guides that tell you the information you will be asked to provide in the webforms. They can be found at Report a concern or allegation.

It is recommended you gather all necessary information before you begin. You must complete the webform in one session – you will not be able to save the form to submit at a later date.

If you require a copy of your notification please print a copy before submitting the notification.

NOTE: If this report involves family violence or you are worried for somebody's safety please contact Victoria Police. If your report describes potentially criminal conduct, you must report to Victoria Police. Organisations cannot commence your investigation until Victoria Police have given you clearance.
