Reportable conduct web form upgrades set for Monday

Notifications and updates about reportable conduct are made online via the Commission’s web forms at

These web forms are being upgraded the morning of Monday 23 September. Users may experience a short disruption and not be able to access the system.

Changes are aimed at supporting organisations to provide more information upfront to assist the Commission perform its oversight role and to reduce follow up questions.

Forms being upgraded are:

  • Notification (3 day notification for organisations and notifications from the public)
  • Investigator update
  • 30-day update
  • Investigation outcome update
  • Other information update.

A new head of organisation nomination web form will also be available for organisations that do not have a CEO or principal officer.

Forms can be submitted as usual before and after the upgrade – there is no need to wait for the upgrade.
